Overview
Outreach Case Manager, Cradles Program at The Council on Recovery. Reports to: Cradles Program Manager. Hours: Full time with some weekend and evening hours. Travel locally – Mileage reimbursement provided. Status: Non-Exempt. Date Available: ASAP.
The Council on Recovery is a 501(c)(3) not-for-profit organization whose mission is to keep our community healthy, productive and safe by providing services and information related to alcohol, drugs, and related issues.
The Role
The Outreach Case Manager (OCM) provides community-based intervention, outreach services and evidence-based education to parents with children under the age of six or expecting parents who have one or more risk factors for substance use. The OCM works one-on-one with parents to evaluate needs and risks, develop individualized strength-based service plans, offer case management services, and create space for families to engage in fun, healthy, sober-free experiences. Responsibilities
- Provide community-based intervention and outreach services to parents with young children or expecting parents at risk for substance use.
- assess needs and risks, develop individualized service plans, and offer case management services.
- Create opportunities for families to engage in healthy, sober activities and supports.
- Collaborate with partners and community resources to support client needs.
- Maintain client engagement with a client-centered and culturally competent approach.

Requirements
- Bachelor's degree in Social Work or related field, or LCDC-I or LCDC (strongly preferred).
- Minimum of two years in Behavioral Health, including substance use experience.
- Minimum of two years' case management experience.
- Experience working with vulnerable families/teens in the substance use field.
- Working knowledge of client/community resources and experience in building partnerships.
- Experience leading groups with vulnerable populations.
- Ability to work independently and as part of a team; valid driver's license, automobile insurance, and reliable transportation.
- Bilingual (preferred).
