Hiring for Director of Social Services

Here at Fall Creek Rehabilitation and Healthcare Center we are searching for an experienced, licensed, and organized Director of Social Services. The perfect candidate will identify and provide for each resident's social, emotional, and psychological needs, and the continuing development of their full potential. You must also be proficient in PCC and discharge planning with at least two years of experience in long term care. Pay starting at $60,000+ depending on SNF experience.
